Skilled Nursing
Our Skilled Nursing Services are provided by Registered Nurses dedicated to meeting the needs of our patients. All home health care is provided under your primary care physician in the home setting of your choice. Services provided may include:
- Blood pressure & Glucose Monitoring
- Disease Management
- Medication Teaching
- Administration of Injections
- Pain Management
- Infusion Therapy
- Post-Surgical Care
- Wound Care
Physical Therapy
Our licensed physical therapists have a depth of experience in helping patients recover from fall injuries, joint replacement surgery and stroke-related disabilities. Physical therapy services will assist the patient in learning how to restore function throughout gait and mobility training; use of assistive devices, artificial limb, energy conservation techniques and exercises.
Occupational Therapy
Occupational therapy services assist the patient in restoring their ability to perform activities of daily living such as bathing, grooming, dressing and toileting, to the prior level of independence and function. A fall risk assessment and a home safety evaluation is performed upon admission in order to make recommendations and enhanced safety.
Speech Therapy
Speech therapy services to assist the patient in restoring language and communication skills to improve quality of life.
Medical Social Worker
Medical social workers consult with the patient regarding financial, social, and/or transportation problems and help establish long-term plans, crisis intervention, and counseling.
Home Health Aide
In conjunction with skilled nursing services and rehabilitation services, trained and certified home health aides provide assistance with bathing, dressing, light meal preparation, and minimal household tasks on a temporary basis.
